Centre for NAI – Intake & Screening (Practitioner Use)
Purpose: Decide to proceed, pause, or refer out. Non-clinical wellness/creativity scope.
1) Explain scope & consent
- Non-clinical; not therapy/medical care.
- Client can pause/stop anytime; no hidden suggestions/recording.
- Reversibility: rollback available on request.
2) Client info
- Name, contact, emergency contact (per local standards).
- Brief goals (calm, focus, creative support, other).
3) Screen for contraindications (if yes → refer/decline)
- Acute crisis/suicidality, recent severe trauma episode.
- Psychosis, uncontrolled dissociation, major cognitive impairment.
- Current severe substance intoxication.
- Meds/conditions where practitioner is not qualified—when in doubt, refer.
4) Red flags / refer out
- Any clinical needs outside practitioner scope.
- Requests for outcomes that sound clinical/therapeutic.
- Signs of instability, mania, severe distress today.
5) Proceeding? Confirm:
- Informed consent signed/acknowledged.
- Safe word/gesture set; no hidden recording.
- Client understands rollback option.
- Agree on focus: supportive, brief, respectful companion only.
6) Notes
- Session notes stored securely if kept; delete recordings on request if applicable.
- If distress emerges: pause, ground, consider rollback, and refer if needed.
